Lobotomy is a fully cooperative scenario crawler in which one to five patients attempt to escape an asylum transformed by their fears. Each character has a personal history, attributes, disorders, phobias, and abilities that shape how the group explores the modular hospital. Action points are spent moving, searching, interacting, and fighting; custom dice turn attacks and defenses into volatile pools while equipment, memories, and skills gradually make the patients more capable. Scenario layouts and objectives change which wards matter, and the monsters draw on pulp horror rather than a single mythology. Time is a persistent threat because the Warden advances while the group investigates, forcing a choice between gathering enough resources to survive and pushing toward the escape condition. The tone is deliberately lurid and surreal, but beneath it sits a demanding cooperative puzzle about positioning, action economy, threat control, and using sharply different characters as one team.
